OFFICE POLICIESOffice HoursOur office hours are 9 AM - 5 PM Monday through Friday. Our main office is located at 3400 Nesconset Highway, Suite 101, E. Setauket, NY 11733. Our satellite office is located at 6144 Route 25A, Suite D21, Wading River, NY 11792. To schedule an appointment please call our main office at 631-751-8700. Click here for directions to our offices. Initial ConsultationOnce you schedule an appointment, you can save time by downloading our New Patient Forms and completing them prior to your arrival. If you are an existing patient, please access our Interval History form here to update your records. Also, if you need to update your address, phone number, or insurance information, please access our Change In Patient Information form here. Please bring these completed forms with you on the day of your scheduled visit. Additionally, please bring any pertinent bloodwork results, as well as previous endoscopy or x-ray results, to the office at the time of your visit. InsuranceIn an effort to accommodate our patients, Long Island Digestive Disease Consultants participates with most major insurance carriers. Below is a partial list of major carriers with whom we participate. From time to time, this list may change. If you do not see your plan listed, or if you have any questions about your insurance, please contact our Billing Department for additional information.
All insurance coverage is verified prior to your appointment. If your insurance plan requires a referral, this must be received by the Billing Department no less than 48 hours before the visit. All co-payments are collected at the time of service and are payable by cash, check or credit card. Please remember to bring your insurance card and driver’s license to the appointment for identification purposes. It is in your best interest to understand the details of your individual health policy. Our Billing Department is happy to assist you with any questions you may have. Emergency ServiceIn the event of a gastrointestinal emergency after hours, please call our office at 631-751-8700 and provide our answering service with all of your information. They will contact the on-call physician who will call you back. If you feel that your problem requires more rapid attention, please dial 911. Medication Renewal PolicyIf you need a prescription renewed, please call our office approximately one week prior to running out of your medication. We will refill your prescription promptly, however by calling us ahead of time, a lapse in your medication will be prevented. Authorizations for ProceduresIf you are being scheduled for a procedure, our Billing Department will take care of any necessary preauthorizations with your insurance company prior to the procedure. We will notify you only if we encounter a problem. However, as a patient, you are responsible for knowing the specifics of your health plan, up to and including, referral authorizations and precertifications. Hospital AffiliationsWe are affiliated with the following hospitals: John T. Mather Memorial Hospital - Port Jefferson, New York Medical RecordsIf you require a copy of your medical records, please be advised that there is a fee of .75 per page for all records provided. Cancellation PolicyWe understand that situations arise which may prevent patients from keeping scheduled appointments. If it is necessary for you to cancel an office visit or procedure, please notify us within 24 hours of the scheduled date. |
